About the Role

An exciting opportunity has arisen for an experienced vibrant registered nurse who is seeking to make the next step in their palliative career to join our team based at St Mary’s Hospital (IW NHS Trust).

As our Mountbatten Palliative Care Discharge Coordinator, you will actively support discharge planning and facilitation for palliative and end of life care patients. You will help to prevent inappropriate admissions into St Mary’s Hospital and ensure that care at home is a viable option for people approaching the end of life.

You will be working in partnership with Social Care, Primary Care, NHS Providers and NHS Continuing Healthcare in facilitating integrated patient-focussed service provision. Whilst an employee of Mountbatten, you will be a core member of the Integrated Palliative and End of Life Care Team within St Mary’s Hospital.

You will ensure communication links are in place to deliver a continuous, well-coordinated and seamless discharge process, as well as tracking those patients appropriately admitted into the hospital.

About you

You will be a Registered General Nurse with at least 3 years’ experience in an acute hospital or palliative care setting.

You will have excellent communication skills which will enable you to work sensitively with patients and families at a very difficult time in their lives. You will be an effective advocate for your patients and will understand the needs of those receiving palliative/end of life care as well as governance issues, risk management and audit procedures.

Previous experience of collaborating with other health and social care providers in primary, secondary, independent and voluntary care is essential.

You will have excellent organisational and negotiation skills and a detailed knowledge of care needs documentation, e.g. health needs assessments, continuing health care assessments, mental capacity assessments, etc
