About the Role

This post is working in the John Cheverton Centre at Mountbatten, providing Nursing assessment, review and interventions for individuals with a palliative diagnosis. You will provide individualised, person-centred care for patients accessing the centre to enjoy it’s many welcoming and supportive facilities, promoting a culture of empowerment and reablement to support them in living well with their condition.

  • Provide Nursing assessment and reports of the current patient status, support patient and carer, and initiate and deliver specialist interventions as required.
  • Work autonomously and also as part of a team with Nursing, Psychosocial and Therapy Colleagues to provide holistic care with a proactive approach to identify and address Palliative Care needs early.
  • Provide or recommend appropriate symptom management, understanding the physiological, psycho-social, Spiritual and cultural needs of patients and their families/carers.
  • Delegate tasks/duties appropriately to support workers and volunteers, considering role boundaries, experience and skill level.
  • Support the development of the John Cheverton Centre in employing innovative and creative solutions to increase Mountbatten’s reach and support our Community.

About You

The post holder will possess first level registration with NMC

Ability to work autonomously and as part of a team, forming good working relationships at all levels.

Good organisational skills, ability to prioritise and make effective decisions.

Person-centred approach with insight and understanding of the value of empowering and enabling individuals with a palliative diagnosis.
