VOICES Survey

At Mountbatten we are proud to have history of providing outstanding care and support to our local community for almost 40 years, and we rely on your opinions to help us improve. This questionnaire is about the care received by people from Mountbatten, as well as the care given to their family and friends. By taking the time to share your experiences with us, we can effectively monitor our services to make sure that the care we give is as good as it can be. As you go through the questionnaire, please follow the instructions and answer the questions by ticking the most appropriate box. If you would rather not answer one or more of the questions, please go on to the next one. We are very interested in what you have to say - please add comments in the free text boxes if you would like to add more to your answers. Thank you for your time.
