You will probably have been referred to Mountbatten by your GP or other health professional.

Once we have your details, we will contact you to arrange an appointment with one of our Mountbatten nurses. If you are able to travel, this is likely to take place in the John Cheverton Centre at Mountbatten Hospice. However, they can also visit you at home and, together with your family or carer, make a plan with you about your future care.

There are many ways Mountbatten can support you and your family, depending on what you need.

  • Initial and ongoing assessment of your needs by an expert Mountbatten professional
  • A full plan that is individual to you, where we will work with you and those close to you to express and record your wishes for the future
  • A telephone advice and support line 01983 533331, available 24 hours a day, seven days a week. This is available both to you and to those who are caring for you, and can be called at any time of the day or night to ask for help and support
  • Access to our Mountbatten Volunteer Service, which may be able to provide a volunteer to visit you and support you at home
  • Access to some of our activities at the John Cheverton Centre
  • A home visiting service for those people who need urgent care and support.
