When you have been referred to the Mountbatten Coordination Centre, and with your permission, your information will be added to the Share My Care Register. This is an electronic record of your decisions about:

  • The type of care you would like to receive as your illness changes or progresses, including as you approach the end of your life.
  • Your preferences for where you would wish to be cared for if you are unwell.

The Share My Care Register also holds key information about your diagnosis, your illness and any treatment you are receiving.

It can only be accessed by the professionals who are responsible for your care, such as your GP, District Nurses, Specialist Nurses and Mountbatten Coordination Centre staff.

All of the professionals involved in your care can access the register at any time of the day or night, so you can be confident that everyone looking after you knows about your wishes and future plans.
